Community Referrals and Concerns
At Lewis University, we are committed to providing clear pathways for students, employees, and community members to voice their concerns, seek support, and access resources. This guide outlines the appropriate channels to ensure your concern is addressed with care, confidentiality, and respect.
STUDENT CONCERN: Are you a student?
- Are you concerned about an academic matter (appeal of final grade, academic dismissal, appeal of another academic matter, concern about classroom behavior)?
- I need help with one or more of my classes. (Academic Support Resources)
- Are you concerned about the behavior of another person?
- I need help with something else.
- Do you have a general complaint, which is an expression of dissatisfaction concerning a University employee, department, service, or process, or a University administrative action, that requires clarification, investigation and/or resolution? (Student Complaint Form)
- Do you have a suggestion for the Student Senate to help make campus a better environment for students? (Student Senate Suggestion Box)
- Is there anything we can do to further support you?
FACULTY/STAFF CONCERN: Are you a Lewis employee?
- Are you concerned about a student(s)?
- Are you reporting an academic concern about a student (attendance, needs academic support, advising question or issue) (Success Coach Intervention)
- Are you reporting a violation of Academic Honesty? (Academic Honesty Form)
- Are you reporting a non-academic concern about a student (mental health, food insecurity, financial challenge?) (Report a person of concern)
- Are you reporting a situation in which you feel a student could present a possible danger to themselves or others? (Report a person of concern). If there is an imminent threat, please call LUPD at 5911
- Have you witnessed or been subject to behavior that violates University policy/Student Handbook involving a student(s)? (Incident Reporting Form)
- Do you have a different concern about a student (Report a person of concern)
- Are you reporting a concern involving sexual misconduct (Sexual Misconduct Report)
- Have you witnessed or been subject to bias, discriminatory or harassing behavior by a student Incident Reporting Form)
- Are you concerned about another employee(s)?
- I witnessed or was subject to concerning behavior by a faculty or staff member (Report a person of concern)
- I have witnessed or been subject to behavior that appears to be bullying or abusive (HR Form)
- I have witnessed or been subject to behavior that appears to be biased, discriminatory, or harassing. (HR Form)
- Are you reporting a concern involving sexual misconduct? (Sexual Misconduct Report)
- Have you witnessed or been subject to behavior that violates University policy? (HR Form)
