Aviation Safety Action Program (ASAP) Reports
The ASAP Report should be used for any incident or observation involving any unsafe act or condition.
The Irregularity Report should be used for any incident or observation involving a rule or policy not being followed, or if an occurrence seems incorrect.
The Suggestion/Complaint Report should be used any time there is a complaint against a person(s) for any behavior, or a recommendation to improve a practice or procedure.
