If you believe that the conduct of an employee was inappropriate, you may request to speak to a supervisor with the Lewis University Police Department.
If a supervisor is not available or if you prefer, you will be given a Complaint form to take with you. Fill it out with as much

By Illinois law, the complaint form must be notarized. You will be contacted when the complaint is received.
