Filing a Complaint
If you believe that the conduct of an employee was inappropriate, you may request to speak to a supervisor with the Lewis University Police Department.
If a supervisor is not available or if you prefer, you will be given a Complaint form to take with you. Fill it out with as much 
detail as possible, and return it to the Lewis University Police Department. You may return a completed form by:
- Mailing the form to the Lewis University Police Department.

- The Main Office. The Department's main office is located in the lower level of the Student Union, north of the Flyer's Den.
If you are unable to pick up a form at any of these locations, you may also download a form from the University Police website. You can click on the graphic to access a printable version. When you have completed the form please return it to the Lewis University Police Department.
By Illinois law, the complaint form must be notarized. You will be contacted when the complaint is received.
