Immunization Policy
In accordance with Illinois state law, students are required
to provide the Center with proof of immunizations prior to enrollment.
Students not in compliance with the immunization policy will incur
fines.
International students must have proof of health insurance
in an amount not less than ten thousand dollars U.S. ($10,000).
The insurance carrier must have a U.S.-based office. International
students who cannot provide proof of
health insurance from a U.S.- based company are required to purchase health
insurance through the University.
Meningitis
VaccineThe
Center
for Disease Control recommends that freshmen living
in residence halls receive
a meningitis vaccine. You may receive this
from your private
physician or Public Health Department. Each
fall, a private healthcare company
will be offering Lewis students a chance
to receive this vaccination on campus. You will receive information
regarding this
special
clinic prior to the beginning of the semester.
Report of Medical History
To assist Health Services in providing
medical services,
students must complete a medical questionnaire
detailing past medical history. This form was enclosed
in your
admissions
packet.
In addition,
you may submit a physical examination
report from your physician,
if you like.
Insurance
All students are strongly
encouraged to have health insurance. Lewis University
provides accident
insurance for its full-time resident students
and student athletes. Information
on health insurance available for commuters,
as well as insurance
for all students, is available at the
Center for Health and Counseling Services and the Office of Student
Affairs.
Proof
of health insurance
is required upon arrival of all international
students.
International
students without health insurance will
be required to purchase the policy through the University.
Diet
Waivers
Resident
students requiring a special diet should
provide evidence
of
their
medical condition (laboratory reports,
x-ray reports, evidence of hospitalization,
etc.) warranting the special diet.
The student's physician must submit a copy of the diet. Only
if the
University
Dining
Services are unable to satisfy the requirements
of the diet
will Health
Services authorize a waiver of a
required meal plan. Diet waivers will only be granted during the
first
two weeks of a
semester
and
are only good for that particular
term. A waiver
request must
be completed each semester.
